Gnathology and cranio-mandibular orthodontics
Effective treatments for a correct bite and the health of the temporomandibular joint
What is gnathology?
Gnathology is the branch of dentistry that studies the function and structure of the masticatory system — teeth, temporomandibular joint (TMJ), muscles and craniofacial bones. The main goal is to maintain or restore the balance between these elements, ensuring efficient, pain-free chewing and a correct jaw position. Gnathology analyzes how the mandible moves and interacts with the maxilla, as well as how this process influences the rest of the body.
When should you see a gnathologist?
- Pain or discomfort in the jaw or joint (near the ear)
- Clicking or popping sounds when opening or closing the mouth
- Locking or limited mouth opening
- Headaches, neck pain or shoulder pain
- Bruxism (teeth grinding), especially at night
- Pressure in the ears or ear pain

How do we diagnose cranio-mandibular dysfunctions?
Examination of the face, oral cavity and posture
The specialist evaluates how the teeth fit together (occlusion), how the jaw moves and whether there are signs of tooth wear (for example, from bruxism). Body posture is also analyzed, as the position of the head and neck can influence the temporomandibular joint (TMJ).
Palpation
This involves identifying pain points, muscle tension or spasms in the head and shoulder area.
Palpating the joint and surrounding muscles provides valuable information about possible muscle imbalances associated with cranio-mandibular dysfunctions.
Listening to joint sounds
Using a stethoscope or specialized devices, the gnathologist listens to the temporomandibular joint (TMJ) to detect clicking, friction or blockages.
These can indicate changes in joint surfaces or other TMJ dysfunctions.
Gnathology Treatment
Treatment in gnathology is based on a personalized and multidisciplinary approach, tailored to each patient’s needs. The most common methods include:
Occlusal splint (relaxation splint): Helps maintain a correct jaw position and protects teeth from wear, especially in cases of bruxism.
Occlusal rebalancing: May include minor adjustments of the teeth (selective grinding) or the application of veneers/crowns to correct the bite.
Physiotherapy and specific exercises: Help relax the muscles and improve the mobility of the temporomandibular joint.
Orthodontics: In cases of severe malocclusion, braces may be needed to realign the teeth and restore proper bite function.
Relaxation and stress management techniques: Often recommended due to the strong connection between stress and teeth grinding (bruxism).

Very good dental clinic, I can recommend it :)
Causes of cranio-mandibular dysfunctions
Malocclusion (incorrect bite): improper positioning of the teeth can overload the joint and jaw muscles.

Bruxism (teeth grinding): stress, anxiety or unconscious habits can create excessive tension in the joint and muscles.

Postural disorders: incorrect head and back posture can, over time, affect the temporomandibular joint.

Trauma: injuries or accidents involving the face and jaw can lead to joint alignment problems.

When the temporomandibular joint is affected, other problems can occur.
Frequent headaches or migraines
Muscle tension in the neck and shoulders
Sleep disorders, including sleep apnea
Postural problems affecting the spine
Frequently Asked Questions
The duration varies depending on the complexity of the case and the chosen methods. It can range from a few weeks (for minor adjustments) to several months or even up to a year in more complex orthodontic cases.
No. Most procedures are minimally invasive and painless. You may feel slight discomfort at the beginning, especially when starting to wear an occlusal splint, but this usually subsides as you get used to it.
Yes. Treatments can be combined. In some cases, it is even recommended to integrate orthodontics with gnathological treatment to achieve the best results.
As with any treatment, there may be minor risks — from gum irritation to muscle discomfort. After a thorough evaluation, the doctor will recommend solutions that minimize these risks.
If your symptoms include jaw joint pain, joint noises, jaw locking or frequent headaches associated with chewing, a gnathologist is the right specialist to consult first.
